In many who suffer androgen related alopecias, the advancement accedes to the Norwood-Hamilton scale. However other forms of alopecia may present which can introduce confusion as to the validity of follicular  transplantion.

DPA  Diffuse Patterned Alopecia (androgenetic) – can present as diffuse often inexorable follicle miniaturisation causing hair density reduction at the Anterior, Central and Posterior Vertices. This may not equate with the established Norwood-Hamilton classification. The Occipital Region however is usually unaffected and therefore serves as a donor site from which  follicles may be harvested for transplantation.

Diffuse Un-patterned Alopecia (androgenic) also presents as a miniaturisation and eventual loss of hair-bearing follicles over the entire scalp including the occipital region which renders the subject unsuitable for hair transplantation procedures.

Correct differential diagnosis is therefore essential.
